3. Media Release

4. Waiver of Liability

5. Fees schedule (football/cheer accordingly)

6. Playing/Performance Philosophy

7. Parent or Guardian's Valid California DL/ID

8. 2 proofs of residency (must be from approved MBYFL list and match CA driver's license)
**documents must be in color**

9. Player/Cheerleader Birth Certificate



**Physicals will be due by football uniform fitting** Physicals prior to 3/1/2023 will not be accepted**
